What we do
Our community nursing service provides you with the following:
- wound care (simple and complex)
- diabetic care
- catheterisation
- administration of medications (including injections)
- stomal care
- drain tube management
- complex medical care post discharge from hospital and some more complex personal care to you in your own home.
Fees apply for this service. Information about these fees can be found on our Community Health Fee Schedule page.
If you are over 65 years, the cost is $5.00 per visit (capped at $50.00 per month depending on number of visits and individual circumstances).

This service is for any person requiring a district nurse.
Areas we service
Area covered includes Bacchus Marsh and Melton and surrounding areas.
Referrals
Your referral will be from your general practitioner, treating hospital, Regional Assessment Service, Aged Care Assessment service or a self-referral.
