What we do
The Healthy Mothers Healthy Babies (HMHB) Program helps pregnant women and their families stay healthy during pregnancy and after the baby is born. It is a free community health program that gives support and connects women with the services they need.
Services and facilities
Trained health workers, including community nurses or midwives, provide outreach support in clients’ homes or in the community. The program aims to:
- help mothers and babies be as healthy as possible
- make sure everyone gets the care and support they need
- reduce health problems and make services easier to reach.
The program offers:
- one-on-one emotional and practical support during pregnancy and for up to 6–8 weeks after birth
- help finding and using antenatal care, Maternal and Child Health (MCH) services, and other community services
- support to access services like counselling, dental care and allied health
- help with ongoing care from pregnancy through to after the baby is born.
Who this service is for
This service is for you if you are pregnant and live in the City of Melton or Moorabool Shire and you meet at least one of the below criteria:
- under 21 years of age
- intellectually disabled
- financially disadvantaged
- socially or geographically isolated
- involved in child protection or the threat of child protection
- medically complicated pregnancy
- identify as Aboriginal or Torress Strait Islander
- affected by substance abuse
- affected by family violence
- from a culturally and linguistically diverse background.
Additional criteria:
- Client must be 36 weeks’ gestation or less.
- Clients that have already birthed are not suitable for the Healthy Mothers Healthy Babies Program as it is primarily an antenatal program. Please refer to Enhanced Maternal and Child Health Nursing service.

Referrals
Anyone can make a referral. This includes:
- the pregnant woman herself
- doctors, nurses or midwives
- other services like social workers or community agencies.
