Pathway to Good Health Clinic

Supporting the wellbeing and development of children in out-of-home care.

What we do

The Pathway to Good Health Clinic is a free service that supports children in out-of-home care (OoHC) who have complex health and developmental needs. 

The Pathway to Good Health program is a model of coordinated care and health planning for highly vulnerable children. It has been developed and funded by the Department of Families, Fairness and Housing. 

Our team at the Pathway to Good Health Clinic in Melton includes a paediatrician, speech pathologist, and psychologist. 

The role of the Pathway to Good Health team is to provide a comprehensive assessment of the child and develop a health management plan, including any diagnosis, recommended treatment and/or referrals.

The clinic is available to children and young people (0-17yr 11 months old) in Melbourne’s western suburbs and Brimbank Melton who are being actively supported by child protection and not already connected with paediatric care.

Children are referred to the Pathway to Good Health Clinic by child protection services.

  • Melton Health & Community Services Centre (195–209 Barries Rd, Melton West VIC 3337)
  • VACCA (75-79 Watton St, Werribee, VIC 3030).
  • Caroline Springs Western Health Office (228-232 Caroline Springs Blvd, Caroline Springs, 3023)
