Specialised Rehabilitation Program (SRP)

Specialised Rehabilitation Program offers a short term, multidisciplinary, goal-orientated service which aims to assist clients to optimise their functional ability, improve independence and improve their quality of life.

What we do

Our Specialised Rehabilitation Program (SRP) provides a short-term therapy program with goals that include an emphasis on self-management and patient education. This program will be specific to your needs. The length of time you will be involved with SRP is discussed with you and your carer and will take into consideration your goals and capabilities.

Both individual and group therapy sessions can be provided depending on your goals. Our therapists primarily treat you at the centre. Limited therapy by our therapists can occur in your home or a combination of both. You will also have a therapy program for you to complete at home.

There is no cost for therapy. Interpreters are used as required.

Services and facilities

We provide therapy to you if require rehabilitation for:

  • musculoskeletal conditions
  • neurological conditions
  • falling or if you have a high risk of falls
  • age-related changes where there has been an acute change in function
  • chronic pain (focussed on self-management).

Services available include:

  • clinical psychology
  • community integration and leisure
  • dietetics
  • hydrotherapy
  • neuropsychology
  • nursing
  • occupational therapy
  • physiotherapy
  • podiatry
  • rehabilitation consultant
  • social work
  • speech pathology.

The service also provides tailored group programs, including:

  • Cardiac rehabilitation – for people recovering from a heart event or surgery
  • Pulmonary rehabilitation – for people with chronic lung conditions such as COPD
  • Stroke – for people and carers following a stroke
  • Communication – for people who have issues with communicating
  • Chronic Pain – for people experiencing long-term, persistent pain
  • Falls and Balance Groups – for people at high risk of falls

Who this service is for

  • If you are over the age of 18 years.
  • If you have achievable rehabilitation goals requiring short term multi-disciplinary team involvement.
  • If you are medically stable.

You must consent and understand what the service offers. 

You must agree to participate in therapy. This includes being able to attend multiple therapy sessions, attend during work hours and access our centres.

You will not be treated through SRP if you are one of the following.

Eligible for funded therapy

Private services will be recommended if you are: 

  • eligible for TAC / WorkCover / DVA funding (for a single discipline only)
  • residing in a Residential Aged Care Facility
  • receiving services through a home care package.

Require equipment or home modifications but there are no other rehabilitation goals

Areas we service

This service is for residents of the cities of Brimbank, Hobsons Bay and Maribyrnong.

Referrals

Your referral should include:

  • your details including address, date of birth and contact phone numbers
  • the reason for your referral
  • your relevant clinical history
  • list of current medications
  • any risks to clients or staff
  • your contact details.

Once a referral has been received, one of our staff will phone you to organise a first appointment or discuss a more appropriate service to meet your needs.

Locations

Rehabilitation is offered at two sites. Clients will attend the centre that is closest to their residence.

Sunshine Hospital 

SRP is located on the far right of the main hospital entrance. Enter through the glass doors and our reception is on the left.

Williamstown Hospital

The entrance is on Stewart Street. Enter through the glass doors and the reception desk is on the right.
