Community Aged Care
What we do
Community Aged Care services provides support to older people to remain independent in their homes. We provide a range of allied health and nursing services to achieve your goals. All our services will complete an assessment with you that will help identify your needs and build goals to work towards.

We are a brokered service for those with a Support at Home Package. We work with a number of different providers. If you wish to work with Western Health as a brokered service please speak with your package holder and get them to engage with our Support at Home Administration office on 9747 7018. My Aged Care
My Aged Care is a commonwealth program that supports older people, their family and carers access the aged care system. Eligibility requirements associated with the program include being over the age of 65 years or,for Aboriginal and Torres Strait Islanders, being over the age of 50 years.
To access My Aged Care ring 1800 200 422 (use the contact information here). They will support you to check your eligibility for the program and arrange an assessment.
I have been assessed by My Aged Care and have a referral in the portal or a referral code
Once you have a referral and it has been received through the My Aged Care portal our intake team will contact you to arrange an appointment as one becomes available. If you have been given a referral code please contact our Intake team (on 03 9747 7609) to discuss the referral and all attempts will be made to arrange an appointment. At times we do have extensive waits for services or closed services so we may not be able to accommodate the referral at the time. This will be discussed with you.
Services we provide
We provide a range of allied health and nursing services to help you achieve your goals and increase your independence.
We use mixed models of care. Where safe and appropriate, we offer services via:
- phone
- telehealth (video)
- face-to-face appointments.
Some services can be offered in the home. Please discuss this with the intake officer or your clinician.

Dementia Support Nurse
The Dementia Support program provides advice and support to help clients and their carers manage daily challenges and improve their independence. We also help you to access the services and supports you need to improve quality of life. Group programs are also available to support clients and/or their carers.
Transport Services
What we do
Volunteer drivers provide door to door transport for eligible consumers to attend medical appointments, shopping, and social events.
Cost
The cost for the service starts at $15.00 and increases depending on distance travelled.

What we do
Volunteers make a weekly call to provide a community connection and check on your well-being.

Occupational therapy
Our occupational therapists can help make everyday tasks more manageable so you can maintain independence at home. They may recommend aids, equipment or home modifications to improve your safety and comfort.
Speech pathology
Our speech pathologist can help you with communication and swallowing difficulties. This can include advice, education and assistive technology to support your daily functioning and independence.
Podiatry
Our podiatrists provide professional foot care to help improve mobility, prevent injury and manage chronic foot conditions. They can prescribe orthotics and other supportive devices.
Counselling
Our experienced counsellors offer support to improve mental health and emotional wellbeing. We provide counselling in areas such as family violence, grief and loss, youth support, family work and personal life challenges. We also provide a Social Work service.
Nutrition and dietetics
Dietitians provide advice on maintaining a healthy, enjoyable diet that meets your needs. We can help you develop skills to plan and prepare nutritious meals that fuel your daily activities.
Community nursing
Our nurses support your medical needs so you can function more independently. They’re available for both home visits and centre-based appointments, and can help with wound management, continence support, medication advice and more.
Physiotherapy
Physiotherapists work with you to increase physical function, mobility and independence through individualised treatment and rehabilitation programs.
Exercise physiology
Exercise physiologists design safe, tailored exercise plans to improve strength, balance, flexibility and overall health. Both individual and group sessions are available based on your needs.
Diabetes education
Our diabetes educators offer practical advice on managing diabetes, including medication, nutrition and monitoring blood glucose levels – helping you maintain energy and confidence throughout your day.
Health coaching
Health coaches can work with you to set health-related goals, identify barriers and build strategies to manage chronic conditions and improve your overall wellbeing.
Group programs
We offer a wide range of multidisciplinary group programs to support client needs. These include falls programs, exercise programs, Parkinson’s Disease programs, cooking programs, cognition and communication programs and many more. These groups will be discussed with your following your initial assessment.
Continence nursing
Our nurses support you to manage your bladder and bowel health, so you can live more comfortably and confidently each day.
