NDIS services
What we do
The National Disability Insurance Scheme (NDIS) provides funding for supports and services for people with a permanent or significant disability.
Western Health is a registered and experienced NDIS provider. We are passionate about supporting people to live their best life – with choice, dignity and independence.
We use a person-centred approach, which means you’re always at the heart of your care. Your goals, preferences and strengths guide everything we do. Our role is to work with you to make sure the services we provide are the right fit for you.
Whether it’s improving your health, building confidence, developing skills or accessing your community – we’ll partner with you to make it happen.
Services
We provide a range of allied health and nursing services funded through the NDIS. These services can be provided in your home or within our centre depending on your needs.
Continence nursing
Our nurses support you to manage your bladder and bowel health, so you can live more comfortably and confidently every day.
Dietetics and nutrition
Our dietitians help you enjoy food while maintaining a balanced and healthy diet. We’ll support your goals around nutrition, eating habits and meal planning.
Exercise physiology
Our exercise physiologist creates tailored programs to help improve your strength, balance, flexibility and fitness, supporting you to stay active and independent.
Podiatry
Our podiatrists help you manage foot and lower limb health to improve your movement, comfort and independence.
Physiotherapy
Our physiotherapists work with you to improve your mobility and physical function, through individualised treatment and rehabilitation programs.
Residential support at Grant Lodge
Grant Lodge is a welcoming Residential Care Facility in Bacchus Marsh. We can support eligible NDIS participants who wish to live in a supported, home-like environment with high-level nursing care.
Speech pathology
Our speech pathologists support you with communication and swallowing difficulties. We’ll work with you to develop strategies or use assistive tools tailored to your needs.
Service pricing
The National Disability Insurance Agency (NDIA) usually update their Price Guide on 1 July each year (and occasionally during other times the year).
The prices for all of Western Health NDIS services and supports are those listed in the current NDIS Price Guide and the Assistive technology, Home Modifications and Consumables Code Guide.
We may charge for travel, cancellations, establishment fees and reports.

We will charge the current price for each support item. When the NDIS Price guide changes, we will tell you about the new prices.
We include the agreed charges and supports and services in your service agreement.
Billing
We will send you or your nominee a bill. This is also called an invoice. We will tell you and write in your service agreement how often and the amount we will bill you for your supports or service. We will also tell you when and how you will be expected to pay this bill.
The following is a list of the common fees we charge for our therapy services:
- Occupational therapy - $193.99 per hour
- Physiotherapy - $193.99 per hour
- Speech pathology - $193.99 per hour
- Dietetics - $193.99 per hour
- Orthotics - $193.99 per hour
- Podiatry - $193.99 per hour
- Exercise physiology - $166.99 per hour
- Continence - $124.05 per hour
- Counselling - $156.16 per hour
- Travel - Max rate 30mins of hourly rate for discipline (for travel to home / community visits to / from participant)
- Documentation - Claimed at the hourly rate of the service support (report writing, sourcing of assistive technology from suppliers).
For Grant Lodge, our aged care facility at Bacchus Marsh, your service agreement will list the fees and what reasonable and necessary needs will be met using supports for your goals and individual circumstances.
Supports will be provided by Grant Lodge as required by the Aged Care Act 1997 and supports provided by the NDIS. The supports required by the Aged Care Act include assistance with daily living tasks. Getting started with us
Once you have your NDIS plan, we can get started. Contact us to register a referral – this is how we connect you to our services.

After your referral is registered, a member of our team will contact you to begin developing a Service Agreement that works for you.
Your privacy and personal information
To support you and help you achieve your goals, we’ll ask for your consent to share your NDIS plan and relevant personal information with our team.
We may contact you by phone, mail or email.

Your service agreement
We’ll work with you – and the people you choose to support you – to develop a Service Agreement. This outlines the services we’ll provide and how they’ll be delivered. It’s a flexible, individualised plan designed around your needs and goals.
You’ll receive a copy of your agreement, and it will be stored securely on your Western Health electronic medical record.
Once the agreement is in place, we’ll schedule your appointments based on the services you’ve chosen.
Advocacy and support
You can ask a family member, friend or trusted person to help you understand your options and speak up for you. This person can act as a support person or nominee.
If you don’t have someone in this role, you can still access help through an independent advocate.

Assessments and service delivery
Each service will complete an assessment with you. This helps us understand what you want to work on and which supports will help you best.
We use mixed models of care. Where safe and appropriate, we offer services via:
- phone
- telehealth (video)
- face-to-face appointments.
You’ll be informed how your service will be delivered.
Reports and progress
We are required to provide updates to the NDIS about your progress. This includes:
- a start of service report
- an end-of-plan report.
