We have rehabilitation beds at Footscray Hospital, Sunshine Hospital and Williamstown Hospital.
Rehabilitation is a service for people who need intensive therapy to reach their goals. This can include therapy, education and strategies. We aim to maximise independence and quality of life.
Our rehabilitation services support you if you have had:
- a stroke or other neurological event
- a new diagnosis of a condition such as Parkinson’s disease or multiple sclerosis
- a major medical episode requiring a long hospital stay.
The Rehabilitation team includes:
- Specialist doctors
- Nurses
- Physiotherapists
- Occupational therapists
- Speech pathologists
- Dietitians
- Psychologists and neuropsychologists
- Social workers
- Podiatrists
We also provide group-based interventions. These programs bring people together to work on shared goals and give you the chance to connect with others facing similar challenges.
Referrals to Rehabilitation
Referrals to Rehabilitation are coordinated by the Subacute and Nonacute Assessment and Pathways (SNAP) team. When in hospital a person's treating team may recommend an assessment by this service to see if they would benefit from a rehabilitation stay. SNAP can also talk about referrals with local health care professionals and can advise the best way to have an assessment for people who are not in hospital.
You are not required to go through My Aged Care for this service.
