Our work focuses on 8 key areas – our strategic outcomes – that guide how we connect Western Health, local GPs and community providers to deliver better, more coordinated care:
- Working together – We partner with GPs, community providers, patients, and Western Health teams to design better, culturally safe care.
- Going digital – We improve the way information is shared and data is managed so care is more connected.
- Listening to voices that matter – We ensure GPs and patients have a real say in how health services are planned.
- Joining up care – We lead and support shared care and integrated care models, helping GPs and hospitals work together to improve patient pathways and strengthen the health system.
- Keeping in touch – We make communication between GPs and hospitals clear, timely, and effective through our GP Newsletter and the North Western Melbourne Primary Health Network.
- Making the system easier – We help providers find the right services, in the right place, at the right time.
- Sharing knowledge – We provide GP education that supports high-quality care in our community.
- Improving through research – We use research, evaluation and quality improvement to understand what works best and keep making care more connected and effective.
Updates to clinical correspondence
Western Health now uses HealthLink for Secure Messaging Delivery (SMD).
We send clinical documents securely through HealthLink and no longer use PulseNet. For more information, visit the HealthLink page on our website.
