Discharge Centres

Our Discharge Centres help ensure you receive the right care and support when you leave hospital and improve patient flow throughout the hospital.

On your day of discharge, you may be transferred to the discharge area, where your ongoing needs for a safe transfer can be organized and coordinated.

The discharge area at Footscray is staffed by two dedicated registered nurses. It is open Monday to Friday, 7.30 – 6.00pm. It is not open on public holidays.

The service at Bacchus Marsh is open regular business hours.

The discharge area can be used for a number of activities on your day of discharge. If you are at Footscray you may be transferred to 2 South if you are:

  • Waiting for medicine to take with you after you leave hospital
  • Waiting for a friend or relative to pick you up
  • Waiting for ambulance transport to take you to another hospital or facility
  • Having a scan or x-ray before you go home
  • Seeing a physiotherapist or other allied health staff member before leaving hospital
  • Waiting for Hospital in the Home to see you before going home
  • Having one last dose of medicine before you leave hospital.

The ward will:

  • Let your family know where you will be going when you leave hospital (for example home, rehabilitation, another hospital).
  • Organise for you to come to the discharge area on your day of discharge
  • Book transport if needed
